Community Connections Map
The Community Connections Map is an interactive tool that visualizes partnerships, projects, and initiatives advancing health equity and chronic disease prevention across Peel Region. It highlights how local organizations, researchers, and community groups are working together to improve access to care, strengthen healthy environments, and support inclusive, community-driven solutions.
The map includes a range of initiatives from across the region, including projects supported through the Network for Healthy Populations’ Catalyst Grants as well as other community-led and partner-driven efforts. It is continuously updated to reflect the evolving network of organizations contributing to better health and well-being for all residents in Peel Region.
By mapping these collaborations, the tool helps partners:

- Identify opportunities for coordination and shared learning
- Highlight community strengths and emerging initiatives
- Foster new partnerships to scale impact
